MORE ABOUT BRAVE'S IMPACT

It's not always easy to spot the signs of depression, anxiety, stress and sadness in our youth. That's why Care Connect+ and several local school districts, with support from THE PLAYERS Championship, have teams up to create BRAVE; a program that encourages youth to:

  • ​Talk about mental health issues early on
  • Be open to receiving help
  • Address social barriers
  • Access behavioral healthcare services at the right time, at the right place
  • Care Connect+ serves as the HUB for behavioral health service referrals for St. Johns County, Putnam, Nassau, and Clay County schools. Using a spoke and Hub model and a unified assessment, the BRAVE program screens families for the most appropriate behavioral health services and support in the surrounding area. We then link the family to one of our behavioral health partners for immediate connection to services.

    Using a holistic approach, we help identify youth in need, connect
    with their families to gain deeper insight, and then use our resources
    to ensure they have access to appropriate support and behavioral
    health professionals.

    2019-2021 BRAVE Data

    • ​Increased the rate of students and families following through with mental health services from 35% to 95%.
    • Connected with families and assigned a Community Health Associate within a 3 day average.
    • Connected and navigated 1071 families.
    • Referred 991 youth to behavioral health providers for services
    • Identified 492 additional needs with 90% of those needs being fully met.
    • Tracked 3,041 total appointments attended.

    We are pleased to announce that the 2nd annual, BRAVE Summit is scheduled for March 30th at The St. Augustine Amphitheatre. The Summit aims at opening a dialogue with teens to address the stigma associated with mental health and encourages teens to learn about how to foster mental wellness. The mission is to provide pathways to support systems for those young people who are struggling or know someone who is struggling, so that they feel equipped to ask for and provide the support that is needed.

    Last year we polled a group of teens and the top responses requested were for information on: anxiety, stress, time management, suicide, relationships and most importantly “how to talk to a friend who is hurting”. We as adults and leaders in the community have a responsibility to continue bringing home the message and inviting our youth to make the change in our world that they will inherit.

    This year, we are looking to make a big impact on our young people by way of the 2nd annual, BRAVE Summit. This will be a free, LIVE event, accessible to all high school students, in Putnam, Clay, Flagler, St. Johns, and Nassau counties. The event will take place at the St. Augustine Amphitheatre. We are excited to provide free food, entertainment, music, and speakers all centered around the singular message of breaking down stigma and talking about the issues affecting teens the most.
